Clerical Error in Day and Month in the Date of Birth

Republic Act No. 10172 (RA 10172) further authorizes the city or municipal civil registrar or the consul general to correct clerical or typographical errors in the day and month in the date of birth or sex of a person appearing in the civil register without need of a judicial order, amending for this purpose Republic Act No. 9048.

    1. LCR Copy of Birth Certificate
    2. Baptismal Certificate
    3. Earliest School Record (Elementary Form 137)
    4. Medical Record
    5. Medical Certification
    6. NBI Clearance
    7. Police Clearance
    8. Clearance Certificate of no pending administrative or criminal case from employment (if employed)
    9. Affidavit of Unemployment (if not employed)
    10. Marriage Contract (if married)
